RMR uses this form for its annual on-site review of each CRP’s client case files.  At least 20% of cases, chosen at random, are reviewed page-by-page to verify documentation required for the EE program.  

The following are definitions of the required documents:

“Release of information” refers to any release the client has signed in order for the CRP to obtain necessary diagnostic information from VR or from medical providers.

“Diagnostic information” refers to any information describing the client’s disabling condition. Examples include medical reports, psychological evaluations, and vocational assessments.

“Service Agreement” refers to the RMR form that the CRP must complete in order to obtain permission to begin providing a client with EE services.

Each status 52 or 60 client’s original VR “Individual Plan of Employment” (IPE) must be in his/her file along with any IPE Amendments. RMR’s “Extended Employment Plan” (EEP) or a similar CRP employment plan must be part of the file of status 51 clients. 

“Billing information” refers to CRP forms documenting the dates and duration of services provided.

“Case service records” refers to documentation of services provided to the client. This documentation may also include billing information.

“Annual review of the IPE or EEP by the CRP” refers to either RMR’s Extended Employment Plan dual-purpose form or a similar CRP form, completed to show the outcome of the CRP’s annual review of the client’s work goal.

A “client satisfaction survey” is any survey that the CRP uses to assess the client’s satisfaction with services provided by the CRP.

“Referral form, Part C Decertification” refers to an RMR Referral form, completed for any client decertified from the EE program.
