Extended Employment Plan (EEP)

This is a dual-purpose form that can be used for an extended employment plan and/or for a CRP’s annual review of EE services provided to a client.

The EEP can be used as a plan for maintaining employment in a follow-along or crew setting or as the initial plan in a sheltered-based setting. The CRP authors the EEP with the client’s input.

The EEP can also be used for the annual review of the appropriateness of the client’s employment goal. Again, the CRP is required to have the client’s input.

The EEP can be used to delineate a strategy for maintaining a client’s employment in a community-based job or as the initial plan for employment in a sheltered employment setting.  An EEP is written by a CRP representative with the client’s input.  The same form can be used for the CRP’s annual review to determine the appropriateness of the client’s employment goal.  Once again, the CRP must have the client’s input when making the review.

If a CRP has its own employment plan or annual review form that requires the same information as its counterpart on the EE website, then the CRP may use its own form.
